Yes, we did goof up an order during peak hour, packed a wrong item which was also half-cooked.

This happened on a rainy day when the batter just wouldn't ferment and chef wasn't in for a baking soda overdose.
We ended up serving idlis that resembled Dwayne Jhonson.
